Co-Management and Optometrists in Indiana and Kentucky and Illinois

What To Expect


  • Once a patient undergoes eye surgery and completes their 1-day post-operative exam, the surgeon will, if medically appropriate, formally transfer care to the co-managing optometrist.
    A detailed transfer-of-care notice will be sent to your referring office for each eye.

  • The patient returns to your practice for post-operative care shortly after the transfer.
    Your office should complete a record of the patient’s initial visit following the transfer and provide it to the surgeon for their files.

  • All surgery-related post-operative care provided by your co-managing optometrist is included at no extra charge for up to 90 days after the procedure.
    Any unrelated visits or complaints may follow standard billing practices.
