Dry Eyes -Spanish and English

It sounds crazy to have watery or wet eyes and have your doctor tell you they are dry. Watery eyes, burning, and a feeling of foreign body or debris in your eyes are all symptoms of dry eyes. After getting such a history, the next step after diagnosing Dry Eye Syndrome is to find the response to a series of drops (artificial tears). This is called dose response. Over a period of one week, put one drop of artificial tears in each eye, every two hours, while awake. If your symptoms lessen, or even disappear, it is almost diagnostic that you have dry eyes. If you are not sure, stop using the drops. If your eyes get worse without using the drops, then you have dry eyes. The second week, see if you can decrease the frequency of using the drops, reduce the dose but not less than three or four times a day. (use them every 6 hours this time) The key to using artificial drops for dry eyes is to use them at regular periods, and to use them even if symptoms have disappeared. Waiting for symptoms to appear before putting in drops is like waiting for symptoms before brushing your teeth: It doesn’t work that way. Most people find that they need to apply drops very frequently to find improvement. We can reduce the number of drops we need to use each day by blocking the channel through which tears leak out of your eyes. This procedure is called “plugging,” or plugging the natural tear drainage ducts. These ducts are in your eyelids. Plugging can be done temporarily with collagen plugs that dissolve. They provide help for dry eyes. If taping doesn’t make it better, but it doesn’t make it worse either, then closing the tear duct permanently (with another procedure – cautery) will usually allow you to use fewer artificial drops to get acceptable relief from dry eye symptoms. If necessary, a medicine called “Restasis” (these are drops) can be used, we also have Xiidra and Meibo which are prescription medications. These medicines increases your own tear production or decrease inflammation or decrease evaporation. It may take up to three months (for Restasis) to work, and once you start it, it is usually used for life.  The advantage of plugs is that there is no foreign substance, drug or chemical interacting with your eye.  You own natural tears provide lubrication.

Understanding Dry Eye Syndrome: When Watery Eyes Are Actually Dry

It may seem counterintuitive, but watery or teary eyes can actually be a sign of dry eye syndrome. Along with excessive tearing, symptoms like burning sensations and the feeling of having something in your eye are common indicators of this condition.

Diagnosis and Initial Treatment

After discussing your symptoms, your doctor will likely recommend a trial with artificial tears. This process, known as dose response, helps confirm the diagnosis:

  1. For one week, apply artificial tears to each eye every two hours while awake.
  2. If your symptoms improve or disappear, it’s likely you have dry eyes.
  3. If unsure, stop using the drops for a few days.
  4. If symptoms worsen without drops, this confirms dry eye syndrome.

Adjusting Your Treatment

In the second week, try reducing the frequency to 3-4 times daily (every 6 hours). The key is to use drops regularly, even when you’re not experiencing symptoms. Think of it like brushing your teeth – you don’t wait for cavities to start your oral hygiene routine.

Advanced Treatment Options

If frequent drop application is necessary, your doctor may suggest:

  1. Tear Duct Plugs: These block the ducts that drain tears from your eyes, helping to retain moisture. They can be temporary or permanent.
  2. Prescription Medications: Drops like Restasis, Xiidra, or Meibo can increase tear production or reduce inflammation. These may take up to three months to show full effect and are typically used long-term.

The advantage of plugs is that they work with your natural tears without introducing foreign substances to your eyes.

Key Takeaways

  • Dry eye symptoms can paradoxically include excessive tearing.
  • Regular use of artificial tears is crucial, even when symptoms aren’t present.
  • Advanced treatments like tear duct plugs or prescription medications may be necessary for some patients.
  • Finding the right treatment often requires patience and close collaboration with your eye doctor.

Remember, every case of dry eye syndrome is unique. Your ophthalmologist will work with you to find the most effective treatment plan for your specific needs.
