Botox® is the brand name of botulinum toxin type A that has been in use since about 1990. Dr. Malitz has over 25 years experience with the use of Botox®. In 2001, botulinum toxin type B was introduced under the brand name Myobloc®. Both forms of this naturally occuring substance, Botox® and Myobloc®, are injected into the area to be treated. The medications partially weaken the nerve to the muscle for about four to six months. BOTOX® has many uses although the drug received FDA approval and has been used by trained ophthalmologists since 1989 after the approval in the USA by the FDA for for the treatment of blepharospasm associated with dystonia, including benign essential blepharospasm or VII (facial) nerve disorders in patients 12 years of age and above. Also in 1989 BOTOX® was approved for the treatment of strabismus (abnormal eye movement) in adults.

In 2000, the FDA approved BOTOX® for the treatment of CD in adults to decrease the severity of abnormal head position and neck pain associated with CD.

BOTOX® has received the greatest attention for its non-approved uses especially:

ᴥ Brow Furrow

Two studies released in early 1999 led to the increased use of Botox for a variety of aesthetic facial and neck procedures, including reducing the turkey gobbler effect on the neck, the prominent lines or ‘banding’ that often appear as a result of a facelift, and for eliminating crow’s feet around the eyes, the line between the brow above the nose, and lines around the mouth.

Facial plastic surgeons have been very innovative in their use of botulinum toxin. Currently, botulinum toxin is most commonly used in the management of hyperfunctional lines. Previously, hyperfunctional lines were the source of much consternation for those that suffered from them. These lines often caused patients to be misinterpreted as angry, anxious, fearful, or fatigued. In the past, facial plastic surgeons only had surgical options in their armamentarium including excision or implantation of fat, collagen, or silicone. These procedures often provided minimal improvement and exposed patients to the risks associated with surgery. Botulinum toxin A injections provide an opportunity to manage these hyperfunctional lines with minimal morbidity. The 3 most common sites for injection are the glabella, periorbital crow’s feet, and forehead areas, respectively.
Hyperfunctional lines (eg, glabellar, forehead, crowfeet, platysma, nasolabial lines)
